  • Must be at least18 years of age
  • Medically and mentally stable
  • Primary diagnosis of chemical dependency
  • If pregnant, due date must be a minimum of three months after anticipated discharge date
  • Interview required if not referred by state agency
  • Appointment required (scheduled, Monday through Thursday, 9a.m. – 2p.m.)
  • Urinalysis

Notes
  • Prescription medications must be labeled by a licensed pharmacy
  • All prescriptions and over the counter medications are secured and made available according to state regulations
  • Only one earring per earlobe is permitted
  • Other body piercing rings or studs cannot be worn
  • Excessive earrings are prohibited
  • While in treatment, no new body piercing or tattoos
  • The purchase of energy drinks is not allowed
